Job Title:                   Administrative Assistant - PROS

Location:                   Venture PROS – 975 Elmwood Ave

 

Salary                          $16.00 - $17.50/hr

Employment       Full Time, 37.50 hours per week, Monday through Friday

 

Provide clerical assistance to the PROS Office Manager. Welcome and monitor all clients and visitors entering the program area. Calculate and manage all billing associated with the PROS program and coordinate with Rochester Rehab billing department.

Key Functions:

 

  • Monitor front door access to the building of all clientele and visitors.

  • Assist Office Manager with all designated routine office tasks on a daily basis.

  • Cover the front office duties daily for the Office Manager as needed.

  • Be fully cross-trained with the Office Manager to provide coverage during their     absences.

  • Calculate the program time of clients on a daily basis.

  • Input the daily participation time of clients into the electronic medical record system.

  • Check insurance coverage for clients receiving services on a weekly basis.

  • Complete all billing agreements with newly admitted clients.

  • Communicate with clients regarding any outstanding balances and develop a payment plan.

  • Resolve suspense codes for billing.

  • Reconcile all billing encounter sheets to ensure accuracy.

  • Adhere to Company policies and procedures to include being aware of the importance of Corporate Compliance Policies and the Code of Conduct. Staff is responsible for maintaining required certifications and licenses.

Job Title:              Care Manager

Purpose: To provide patient-centered, high quality care management services in a timely manner to clients with complex chronic conditions by navigating systems such as primary medical, specialty and behavioral health care, as well as social services.

 

 

Key Functions:

  • Provides comprehensive care management services including: outreach and engagement; assessment/reassessment; development and implementation of initial and revised individualized care plans; consult with primary care physician, and other care provider(s); identify crisis intervention plan.

  • Provides care coordination and health promotion including: collaborate with service providers and health plans to obtain medically necessary care, share crisis intervention(s) and emergency info; linkage to services to support care and treatment goals/plans; patient education ; self-help recovery and self- management; case reviews; advocate for services and assist with scheduling; collaborate with providers to ensure service delivery; monitor, support, accompany (as indicated) patient to/with scheduled appointments; crisis intervention .

  • Provides comprehensive transitional care including: follow up on notice of ER/hospital/residential/rehab setting admission and discharge; facilitate discharge planning with ER/hospital/residential/rehab setting staff to a safe location with care needs in place; notify service providers of admission/discharge and follow appointment needs; link with community supports; post discharge follow up.

  • Provide member and family support including: care planning with patient/family; patient/family consultation and education around health care needs; patient/family meetings; patient/family referral to peer supports, support groups, social services and other programs as indicated; collaborate with community providers to support service utilization based on patient/family need.

  • Provide referral to community and social support services including: identifying and linking to community supports as needed; assure and assist with access to medical, behavioral health, substance abuse and other community based services as needed.

 

  • Timely and comprehensive completion of documentation including notes, assessments, tracking tools, etc.

  • Cleans bathrooms, offices, hallways, and any other assigned areas by vacuuming, sweeping, mopping, scrubbing and polishing.

  • Dust furniture and fixtures.

Job Title:                   Multi-Skilled Facilities Specialist

Responsible for performing repair and upkeep, preventative maintenance, and tracking of facilities improvement for Rochester Rehabilitation Center sites.

 

Essential Functions

  17. Purchases maintenance materials for program and works with Finance Department as appropriate to secure supplies.

  18. Maintains vehicles and ensures registrations, inspections and vehicle upkeep and repairs are maintained.

  19. Provides assistance with snow removal and salting as needed.

  20. Completes all documentation requirements in a timely manner.

Purpose: The Administrative Assistant provides administrative support to the Director of Employment Connections as well as to the Director of One Source Solutions to prepare and review reports, maintain departmental files, initiate purchasing for the departments, coordinating all building, computer equipment and phone maintenance.  This Assistant is also responsible for reception duties as the main contact for entrance to the building.  Deals with a diverse group of external callers and visitors as well as internal contacts at all levels of the organization.

 

Key Functions:

Job Title:             PROS Mental Health Advisor

Responsibilities include but are not limited to:

  • Guide a case load of 16-18 clients through the rehabilitation process by assisting each client in establishing and achieving his/her personalized life role goals.

  • Responsible for timely caseload documentation including concurrent development of clients’ individualized recovery plans (IRP), on-going progress notes, IRP reviews, and on-going assessment in accordance with NYSOMH regulations.

  • Participate in on-going development and implementation (teaching) of program curriculum based on clients’ barriers to goal attainment.

  • Communicate with all members of the client’s treatment team to deliver a comprehensive coordination of care.

  • Responsible for the orientation and intake of new clients including comprehensive client assessments.

  • Adhere to Company policies and procedures to include being aware of the importance of Corporate Compliance Policies and the Code of Conduct. Staff is responsible for maintaining required certifications and licenses.

  • Attends necessary trainings and in-services, participates in department and agency meetings as required.

  • Performs other related duties as assigned by supervisor/director.

 

Required Education and Experience:

  • Master’s degree in the human services field (preferred) and at least 2 years of experience in public or community mental health settings serving low income and culturally diverse populations. Bachelor’s degree required.

  • Licensure in New York State as a Limited Permit MHC, LMHC, or LMSW.

  • Demonstrated knowledge of a person-centered planning and ability to work as part of a fast-paced multi-disciplinary team.
