Job Title: Care Manager
Location: Rochester Rehabilitation Center - Elmwood
Salary $18.00 - $22.00/hr

Purpose:
To provide patient-centered, high quality care management services in a timely manner to clients with complex chronic conditions by navigating systems such as primary medical, specialty and behavioral health care, as well as social services.
Key Functions:

Provides comprehensive care management services including: outreach and engagement; assessment/reassessment; development and implementation of initial and revised individualized care plans; consult with primary care physician, and other care provider(s); identify crisis intervention plan.

Job Title: Crew Leader - GSA/Federal Bldg
Location: Rochester Rehabilitation Center – University Ave

Salary: $18.50 – $19.50/hr
Employment: This is a full time 40 hours per week position, M-F 12:00 pm – 8:30 pm
Purpose:
Serves as a lead worker over a custodial crew performing routine manual tasks in maintaining cleanliness, sanitary conditions, and the appearance of assigned building. Supervises, directs and assists in the evaluation of the activities of assigned personnel. Provides training in custodial methods and procedures to supervised staff and assists Operations Manager in performance of time studies. Monitors inventory of cleaning materials associated with the building. Assists with maintenance and repairs of cleaning equipment.

Key Functions:

Provides supervision and assigns work to crew members; demonstrates custodial techniques to make sure assignments are understood; Ensures custodial carts are well stocked and equipment is in good working order; Inspects work performed by crew members to insure assignments are complete and quality standards are achieved.

Job Title: Director of Mental Health Clinic
Location: Rochester Rehabilitation Center – Elmwood Ave

Salary $70,000 - $75,000 per year
Employment Full Time/12 Month, 37.50 hours per week, Monday through Friday
Purpose: Under the general direction of the Vice President, the Mental Health Director, oversees and supervises the operations of the clinical services in the Mental Health programs. The programs include the Mental Health Clinic and oversight of the program manager for Drive On/ Care Management.

Key Functions:

Oversees the provision of clinical services, assures performance of the programs through monitoring of productivity expectations and program budgets.

Purpose: The Administrative Assistant provides administrative support to the Director of Employment Connections as well as to the Director of One Source Solutions to prepare and review reports, maintain departmental files, initiate purchasing for the departments, coordinating all building, computer equipment and phone maintenance. This Assistant is also responsible for reception duties as the main contact for entrance to the building. Deals with a diverse group of external callers and visitors as well as internal contacts at all levels of the organization.
