Job Title:                   Care Manager - FT

Location:                   Rochester Rehabilitation Center – Elmwood Ave

Salary                          $18.00 - $22.00/hr

Purpose:

To provide patient-centered, high quality care management services in a timely manner to clients with complex chronic conditions by navigating systems such as primary medical, specialty and behavioral health care, as well as social services.

 

Key Functions:

  • Provides comprehensive care management services including: outreach and engagement; assessment/reassessment; development and implementation of initial and revised individualized care plans; consult with primary care physician, and other care provider(s); identify crisis intervention plan.

  • Provides care coordination and health promotion including: collaborate with service providers and health plans to obtain medically necessary care, share crisis intervention(s) and emergency info; linkage to services to support care and treatment goals/plans; patient education; self-help recovery and self-management; case reviews; advocate for services and assist with scheduling; collaborate with providers to ensure service delivery; monitor, support, accompany (as indicated) patient to/with scheduled appointments; crisis intervention.

  • Provides comprehensive transitional care including: follow up on notice of ER/hospital/residential/rehab setting admission and discharge; facilitate discharge planning with ER/hospital/residential/rehab setting staff to a safe location with care needs in place; notify service providers of admission/discharge and follow appointment needs; link with community supports; post discharge follow-up.

  • Provide member and family support including: care planning with patient/family; patient/family consultation and education around health care needs; patient/family meetings; patient/family referral to peer supports, support groups, social services and other programs as indicated; collaborate with community providers to support service utilization based on patient/family need.

  • Provide referral to community and social support services including: identifying and linking to community supports as needed; assure and assist with access to medical, behavioral health, substance abuse and other community-based services as needed.

  • Timely and comprehensive completion of documentation including notes, assessments, tracking tools, etc.

Job Title:                   Custodian/Janitor - Full-time

Purpose:          To Perform routine manual tasks in maintaining the cleanliness, sanitary conditions and appearance of assigned buildings.  Follows the contract and quality control procedures

 

Key Functions:

  • Cleans bathrooms, offices, hallways, and any other assigned areas by vacuuming, sweeping, mopping, scrubbing and polishing.

  • Empties and decontaminated trash containers.

  • Dust furniture and fixtures.

Summary/Objective

The Driver Rehabilitation Specialist assists clients in achieving their goals to gain mobility independence, by thoroughly assessing their ability to drive.  Provides driver training as needed to obtain or retain their driver’s license.  The Driver Rehabilitation Specialist also recommends vehicles and adaptive equipment for drivers and safe passengers with disabilities.

Job Title:                   Multi-Skilled Facilities Specialist

Responsible for performing repair and upkeep, preventative maintenance and tracking of facilities improvement for Rochester Rehabilitation Center sites.  

 

Essential Functions

Purpose:

Using a person-centered approach, assist consumers with preparing and seeking successful employment in the community.  Responsible for working across all vocational and employment programs by providing a wide range of vocational/career planning activities including:  job readiness soft skills workshops, career and vocational assessments, skill development and career advancement activities, worksite tours, job placement and retention supports and coaching.  Coordinates with local educational and training institutions, businesses and one-stop center.  Engages with Management team on strategic planning initiatives and billing.

 

Essential Functions

  6. Develops and maintains relationships with local educational and training institutions, employers, businesses, one-stop centers and key community stakeholders. 

  7. Provides proper documentation of all recorded visits and work done and achieves established productivity standards in conjunction with applicable program contracts, goals and objectives.

  8. Works collaboratively within the department and with other internal departments to foster effective working relationships to ensure that participants needs are met.
