Case Management
Case Management is a service for participants in the Mental Health and Vocational Services programs, who need support and assistance with:
- Linkage to housing options
- Financial entitlement programs (Social Security and Department of Social Services)
- Transportation
- Physical and dental health care
- Budgeting
The goal of the program is to promote independent living, self-sufficiency, and assist individuals in improving their quality of life.
The services are tailored to individual needs.
The Staff
Case Management staff includes professionals trained in social work.
How to Get Services
To receive Case Management services, an individual must be enrolled in one of the Mental Health programs or Vocational programs and have a primary mental health diagnosis.
